Unidentified Lighting in Star Trek

Although most Star Trek sets don't have conventional light fixtures, several off-the-shelf lamps could be seen. We already have an article about lighting that is or was commercially available.

The following list contains some notable lighting we couldn't identify yet. Please help us find out where it came from (such as in an e-mail or in the comments). We are looking for exact matches; hints about very similar styles may be helpful as well.
